In response to a few people who have enquired about an Annual Report, here is a sample of one of my own. The Diversional Therapists Annual Report is a wonderful forum by which to communicate effectively to your manager and peers without letting your emotions get in the way.

This sample program is for acute Dementia care. Some people like to do it weekly, others monthly. I tried to have a music program at least once daily. One cannot underestimates the value of music in the lives of dementia clients. Downlod the form and adjust it to suit you. If you have an activity that works well for you I would love to hear it. A monthly program will be available shortly.
The evaluation of social outings can be done in various ways. You may just ask residents whether they liked it and include questions such as “What did you like best?” ‘Would you come back?” or you may keep in mind your objectives for the outing and write about your observations. Remember that any inadequate or difficult behaviour should be passed on to the RN in charge.
Social Outings should be carefully planned, with safety always being the utmost priority. The ratio of carers per resident depends on the capabilities of each resident.
A checklist is the best preparation for social outings. It's also great for piece of mind!
The purpose of an action plan is to improve the recreation and leisure lifestyle of your facility.
The 'Health & Lifestyle Survey' is a tool used to determine whether the respondents have a positive view of the facility and the care being provided. The aim of the survey is to gather information from residents regarding their attitudes towards their health and lifestyle so that you can always strive to improve your services.
For facilities that have people from a cultural and linguistic diverse background (CALD) it is a good idea to make a 'Radio Guide' with musical and talk-shows programs to suit each individual. This is a sample of a Multicultural Radio Guide form.
It is important to support volunteers and show your appreciation. This is one of three certificate of appreciation you can download. The signatures at the bottom of the certicate are for the Manager and Recreation Therapist.
There are a few ways to record programmed actitives attendance. This is an individual one. The advantage of this one is that you can see at a glance how the resident is doing. Evalutaion will be easier if you compare attendance month by month.
One-on-One forms are useful for recording individual visits. Individual visits are interventions used to support residents who are ill, unable to attend programmed recreation or simply refuse to leave their rooms.
