We are looking for a risk assessment. It could be a general one but need to incorporate falls.
We have a care plan thought this could be written into mobility. This is for centre based day care. Any suggestions.

The falls risk assessment needs to be a validated tool as this is required for quality review purposes, I would suggest you look at FHROP - COMM

glad you found it, I was just looking for the link to send to you. Did you get the whole booklet produced by the Australian Commission on Safety and Quality in Healthcare ? Its called A Guidebook For Preventing Falls and Harm From Falls in Older People : Australian Community Care. Its a valuable resource .
