Hi Trisha, great idea! You may find the Activity Evaluation Form Template helpful https://www.goldencarers.com/activity-evaluation-form/5165/. It is a good tool because it allows you to identify the time of day the activity is best-used and offers a comments write-in section that could allow you to write in who the activity benefits and why!
