Hi,
Have any of you set up forms that look into the residents needs and wishes to help with person centred care. I know the 'This is me' forms touch on this, but i mean more detailed from an activity point of view. ie, favourite singers, songs and what these remind them of, favourite recipes and memories of them, hobbie's and interests to help with activity idea's, favourite outings, sports, final wishes etc
