Good morning from Australia,

As I've got to organise Case Conferences for my residents and their families in the facility, I want to create a list of other questions to ask the families and residents besides the clinical side of things.

Does anyone have a template they follow or any ideas?

I also would to create an invitation template for the case conference.

I am from the United States and we had someone else nursing schedule the meetings

Ha ever a few pointers because I want to the care plan meetings

You want to keep the meetings as short as possible because families members tend to talk a long time because of many reasons

One important reason is because they feel guilty and their family member in the facility complains to them by being there

Usually social service has a lot of input in these meetings because they are the ones to resolve family issues

This is not a template but it might give you some ideas
