Hello All, I am looking for a 1 page who am I that the families help fill out when their family members enters a Memory Care Facility. something that tells us what they like, hate, favorite foods, drinks, favorite songs ETC (you get the idea) I have noy t be able to find a fun 1 page that is for senior.
