I did send you a general care plan which could easily be altered. It was made in word so you can easily erase what you do not want and replace it. You can also type in it. You may have to erase some lines however.

I started from scratch and did what you asked. The Activities word I used word art. You can change it to say whatever you want. You can easily type in each column and change the size of each column.
You can easily cut and paste after you save this. Let me know if you need more help
