Hi all, an awesome site, thank you for all the wonderful sharing of ideas. I am wondering if anyone has a list of holistically based questions / prompts that could be used to help Residents think about goals/plans they might like to set for themselves and the support they might need. I have a form where residents are invited to participate in plans around Communicating, Family, physical, emotional, sexual, etc. Often they are stumped and I don’t have any supporting documentation to help me spark their ideas for themselves. Hi Carol, I love the empowerment that your goal setting plan offers. Have you offered a goal setting course? It may be helpful to offer an educational piece before handing out the documents. While I don't have any specific holistic questions to offer I would suggest starting a less informal conversation with each resident to better understand their interest and preferences and build up to setting a goal. I would love to hear how this progresses for you!

It takes the pressure off and allows more time to be spent with the ones we are caring for who after all need our time more than anything else.
