Hi everyone. I was wondering if anyone has a user friendly Pet Application/Agreement form that they use for pet therapy sessions? I have the aged care approved form, but its not very user friendly and I think it deters volunteers from applying. Any help would be greatly appreciated.
Hi Lowana, I can give you a description of a typical form. The Pet Application & Agreement form outlines the role and responsibilities of the pet owner. It contains, among other information: the pet picture, breed, age, dog training certificates, vaccination and microchip or registration details. It also contains references from vets or previous owners, as well as, information on what the pet was trined to do: fetching balls, shaking hands, sitting on command, bark on command, roll over.
