Hi Karla, I'd love to see what others are using too. I googled 'photo release form senior care' and there were a lot of examples you could model off, but it would be great if someone could send in a photo of the one they use.

I have attached the permission release form that I have used for my residents, I have taken all the logos out so all you need to do is place your care home name and logo in. I hope that this helps.
