Good morning from Australia,
As I've got to organise Case Conferences for my residents and their families in the facility, I want to create a list of other questions to ask the families and residents besides the clinical side of things.
Does anyone have a template they follow or any ideas?

I also would to create an invitation template for the case conference.

Hi everyone,
Im a Team Leader for my Household (Tasman) which consists of 20 residents. My facility has 100 residents in total broken up in to 5 HH.
I want to get some ideas where I can create something with the residents where we can bond together to show we aren't just a HH but a very close caring community. Then we can hang it up in our entry to our Household for family and visitors to see.
Any ideas??

Hi everyone, I'm from Australia and I've been in Aged care now for 2 yrs. I've been a "Home Maker" aka Care co-ordinator for about 3mths.
Our facility caters for 100 high/low care residents and I have 40 of those residents on my floor, where I share the floor with another HomeMaker. We have 20 residents on each side. She "manages" her side and I have mine BUT we run our floor together.
We need to start implementing "Community circles" where I bring my residents together sit around in a circle and we discuss their positive and negative concerns.
My question is....
Has anyone else had experience doing this?
Where would I begin?

What a fantastic idea!! Ive been working in Aged Care for 18 mths now and just got promoted to become a shared Care Co-ordinator for one of our floors with 40 residents both high and low care. I absolutely love my job and want to make the residents feel happy and always engaging in different activities. So this website is wonderful for me to get great ideas from. I cant wait to start. :)
