I am after some tips on the on going process of evaluating activities. I would like to evaluate activities 3 monthly. Does anyone have such a form or any ideas they could share on this please.
Hi Cherie I would start by asking the residts at resident Council what they think of activities that you are doing now what they would like you to add and any other ideas they may have You may also want to send a letter to family members or give them a form at a care plan meeting asking for their suggestions To devise a form Think about the different types of activities that you do and think about what you do in each category Then prepare and easy to follow easy to fill out large print form that you can give to resident and family members Encourage residents and family members to fill out the forms Be sure to give them enough time Perhaps you can help some of those have difficulty writing or seeing Then compile the results and see what you come up always good to have if certain residents complain about certain things you can I always show these forms I will see if I can find a form online and get back to you later I hope this helps Susan
Hi there I created a GAP group activity plan. This form consist of: Activity Objectives Where to find equipment Cost. Who attends Where is it held Review 6monthly you can add wick questionnaire with this form and that also ticks another accreditation box. Mine is a little more comprehensive but this is a good start. Cut and paste these ideas as well as some of your current activities that are not well attended and some ideas for new activities
