Diversional Therapy Coordinator
I'm new-ish to Diversional Therapy and have just found out I need to write up a care plan for the animals/pets we have at our facility. Does anyone have a template for a care plan such as this or know where I can find one. I have written out care plans for residents but am just at a bit of a loss as to what to include and how to write it all up professionally etc... Thanks for any assistance.
hi trudi my care plan for animals , first page states the name of the animal it's discription , vet check and state how often your manager wants the animal checked a place to sign when you do so then you state how the animal is going to be looked after who is responsible when and how often what does it get feed and water who baths or cleans how often and the process for this is there anything people shouldn't feed it , also you include it's temperment depending on the animal . i didn't have a good template so i created one if you need help with that i could put something together or have a look on resources in golden carers they may have something i hope this is helpfull good luck
